KRIMS has from its very initiation been involved with reaching out the community at large conducting diagnostic & health camps as well as creating awareness through various programmes to enhance preventive perspective to medicine. Its also regularly imparts training to its doctors as well as staff oto better equip them in a repidly changing and developing field of medicine.

First Pulmonary Rehab center in Central India. Pulmonary Rehabilitation at KRIMS Hospital

As per the international guidelines, management of chronic respiratory disease like COPD & Asthma requires integrated multidisciplinary approach other than the regular medications. This program is known as 'Rehabilitation'. Every COPD Pt. needs rehabilitation as a part of treatment and every hospital should have such facility.

It is recent addition to Pulmonology dept at KRIMS Hospital and regularly implemented twice a week for all Chronic Respiratory patients particularly in COPD, Chronic Br.Asthma & ILD.

Pulmonary rehabilitation focused on exercises related to breathing, endurance, flexibility, muscle strengthening, and airway clearance technique & energy conservation.

It also takes care of patient education, nutrition, sleep, depression, social support & psychological counseling. Duration of the program is 6-8 weeks in which above training and support is given to the patients. It helps to optimize physical & social performance and autonomy.

Till date 20-25 patients are enrolled this rehabilitation program.

The department of Pulmonology, KRIMS Hospitals has organized a CME "Asthma and Allergic Rhinitis - A way Forward" on 20th Dec 2015. The CME was organized as an educational update for post graduate students  of Pulmonary, ENT & Medicine dept. from medical colleges of Nagpur.

The topics of this clinical meet included advancement in Asthma & Allergic Rhinitis like "Serum IgE & Immunotherapy", covered by Dr. Sneha Tirpude, Consultant Pulmonologist , KRIMS Hospitals and chaired by Dr. Meenakshi Ghadge.

Next topic "Pediatric Asthma", was covered by Dr. Radha Munje, Prof &HOD Dept. of Pulmonology , IGMC Nagpur and chaird by Dr. Anju Kadu, Pediatrician.

"Inhalation Therapy & Techniques."was covered by Dr. Sameer Arbat, Consultant Pulmonologist, KRIMS Hospitals & chaired by Dr. B.O. Tayade, Prof. & HOD, Pulmonary Medicine LMH Nagpur Topic "Allergic Rhinitis & Difficult Asthma" was covered by , Dr. Ashok Arbat, Senior Pulmonologist, KRIMS Hospitals, & chaired by Dr. Sanjeev Golhar, ENT surgeon & Dr. Babaji Ghewade, Prof. & HOD JNMC , Sawangi Wardha.

The meeting concluded with a Interactive session conducted by Dr. Mitesh Dave & chaired by Dr. Nainesh Patel, Pulmonologist.

Dr. Sameer Arbat, young, dynamic Pulmonologist from KRIMS Hospitals, is invited for Interventional Pulmonology Fellowship programme at Ospedali Riuniti Hospital, Ancona, Italy. He will be working under Dr. Steffano Gasparini at Ancona, Italy. Dr. Gasparini is Head of Respiratory Diseases Unit at Department of Internal Medicine & Respiratory Diseases.

Dr Sameer Arbat, MD in Chest Medicine has worked as a Senior Registrar at K.E.M. Hospital Mumbai for 1 year, and then joined TATA Memorial Hospital, Mumbai for Bronchoscopy Training program.

He will be working under Dr. Steffano Gasparini at Ancona, Italy. Dr. Gasparini is Head of Respiratory Diseases Unit at Department of Internal Medicine & Respiratory Diseases. He is world authority on Thoracic Endoscopy and Interventional Pulmonology.

Dr.Sameer Arbat will receive his training in Thoracic Endoscopy that includes Bronchoscopy & Central Airways disorders, Rigid Bronchoscopy with Laser Resection, Stenting & Debulking in Tracheal & Bronchial stenosis, EBUS – TBNA, Thoracoscopic procedures & Mediastinal biopsies.

Dr.Gasparini is a member of ERS, ATS, ACCP, AAB, WAB, EAB. He was chairman of the Study Group Thoracic Endoscopy of Italy & author of more than 200 scientific papers published in national and international journals.

Dr Sameer Arbat, son of Dr. Ashok Arbat, Sr. Pulmonologist & CMD KRIMS Hospitals, has special interest in Intervention Pulmonology and aims to bring advanced modalities of thoracic cancer diagnosis and treatment at KRIMS Hospitals.

Krims Hospital successfully treated 4 year old Burnt Patient.

KRIMS hospital feels proud to add another feather to its cap by successfully treating a 4 year old child Mast. Sarthak R. Sahu, a resident of Amravati. Sarthak was admitted to the hospital for a case of 50 % mixed burns, which he suffered from while playing and falling in a boiling cooking oil vessel at home.

He was under the care of Dr. Dinesh Sarda , Consultant Paediatric Surgeon (M.S., M.Ch., DNB, FAIS, FICS, MNAMS) at KRIMS hospital. The overall comprehensive management by Dr. Swapna Khanzode (Consultant Intensivist), Dr. Anju Kadu (Consultant Pediatrician), and the paramedical and nursing staff resulted in the fast recovery and excellent results for the patient.

Sarthak's family is thankful to the hospital and its staff for taking the needed care of him so he could return home with a smile.

The ILD Update 2016 was a huge success and was attended by the top doctors from all over the country.

The Pulmonology department of KRIMS Hospitals, along with the API Vidarbha chapter, Vidarbha Association, jointly organized the ILD (Interstitial Lung Disease) update in Radisson's Blu Hotel, Nagpur, on 21st February, 2016. A number of renowned and prestigious pulmonologists from all over the country attended this conference. International Women's Day 2016

KRIMS Hospital’s renowned and expert young and dynamic neurosurgeon Dr. Akshay Arbat Patil has just been awarded with the Indo-Japanese Microsurgical and Endoscopic skull base surgery fellowship in Japan.

For this he had visited Japan, one of the most advanced country in neurosurgical field, for Microsurgical and Endoscopic Skull base Neurosurgery fellowship.

First he visited the Hiroshima medical University which is one of the best and the advanced Universities in the western Japan. Here he participated and learnt about micro neurovascular bypass in complex skull base tumours.

After that he attended World Skull Base Congress 2016 at Osaka from 14 to 17 Jun 2016. During this conference he also participated in microsurgical cadaveric workshop.

Indo-Japanese Neurosurgery Congress awarded him with the fellowship in microsurgical and endoscopic skull base neurosurgery at Fukushima Medical University Hospital under the world renowned Prof. K. Saito. Where he spent valuable time in learning micro neurosurgical skull base approaches and advancement in endoscopy in neurosurgery. Looking at his expertise and skills he was invited to participate in decision making processes and the ward rounds there.

There he also spent some of his valuable time at Tokyo Medical University Hospital where he encountered finest surgical skill in the Skull Base surgery under Prof. M. Kohno. Finally the last but not the least he got the chance to earn knowledge from Prof. H Sano ( The Guiness Book record Holder) who did maximum number of aneurysm surgeries in the world

This success of Dr. Akshay Arbat Patil has definitely brought laurels not only to KRIMS Hospitals but also the entire Central India.

The CME Conducted in Nagpur on "Tuberculosis: Recent Advances in the field of Diagnostics"

In this CME our Speaker Dr. Sanjay Rajpal has deliver the session on all the latest WHO approved diagnostics test in the field of TB, esp.ly GeneXpert MTB/RIF test which can diagnose TB in Sputum negative Pulmonary TB patients and simultaneously detect MDR TB in 2 hours instead of 12-16 weeks by LJ Culture method.

Other latest TB tests on which he had spoken which includes, MGIT 960, LPA or Line Probe Assays or HAINS MDR/XDR TB Test or Rapid Genotypic Assays.

He had an interactive TB Diagnostic Workshop where TB Diagnostic Algorithms which has been discussed in length by the him along with the clinical case studies & the esteemed workshop on was also interactive to get the updates for diagnosis of Extra-Pulmonary TB and drug resistant TB

Paper and Poster Presentation by KRIMS Hospitals at London, ERS 2016

Pulmonologists Dr. Ashok Arbat and Dr. Sameer Arbat from Nagpur, represented KRIMS Hospitals at ERS International Congress 2016 at London.

Dr Sameer Arbat was invited to present a clinical poster- “A study of Factors affecting Mortality in ILD patients over 2 years.” The study is a retrospective analysis carried out at KEM Hospital, Mumbai over a period of 2 years from May 2013 to May 2015.

Another study titled "Study of Clinical Profile of patients of Obstructive Sleep Apnea and it’s co-relation with Apnea Hypopnea Index (AHI) " undertaken at KRIMS Hospitals was presented on second day of ERS 2016 at London. This study was an analysis of clinical profile of all the patients suffering from Obstructive Sleep Apnea (OSA) who presented to KRIMS Hospitals, Nagpur.

The Pulmonology Department at KRIMS Hospitals is regulary presenting clinical studies at International Conferences. The European Respiratory Congress is an annual event with participation from all over the world. ERS 2017 is scheduled to be conducted at Milan , Italy.

KRIMS Hospitals in collaboration with Jan Seva Samiti successfully conducted the Mega Health check up Camp at Indira Mangal Bhawan,Pandhurna, Madhya Pradesh in the presence of Shri Kamalnathji ( Ex- cabinet Minister & Member of Parliament) as chief Guest on 16/9/2016. Research Article by Dr.Arbat published in the reputed International Journal "ENVIRONMENTAL DISEASE"

We are pleased to announce that a research article on “Purview of allergens through skin test in Central India” by Dr. Ashok Arbat, Dr.Sameer Arbat, Dr.Sneha Tirpude, Dr.Mitesh Dave & Dr.Sukhant Bagdia has been published in the reputed International Journal “ENVIRONMENTAL DISEASE”, Jul-Sep 2016, Vol 1, Issue 3.

The main objective of the study was to evaluate the performance of various aeroallergens by assessment with SPT in Central India population. The conclusion of the study was that the dust mite had the greatest frequency and that humidity levels are critical for the growth of mites, which provides a valuable clue to protect against it. Avoiding the relevant allergens can be a remedy for Allergic Rhinitis patients and can help to reduce the frequency of symptoms in asthmatics.

Right CPR at Right time... Saves a life Unveils a rare post Cardiac arrest Sequelae

Again a situation where performing basics of cardio pulmonary resuscitation has saved a life. It re emphasizes the need for every individual to be thorough in basic life support technique and for all medical professionals to be thorough with advanced cardiac life support techniques. When 23 year old Shivani got admitted to hospital for a minor orthopedic procedure, little did she know that it is going to be a near death experience.

This young dentist was admitted to KRIMS for anterior crucial ligament repair of her left leg and underwent procedure smoothly. Post operatively she was mobilised within 6 hours. Following mobilization she collapsed on the floor and medical emergency team was called in. Chief of ICU Dr Swapna Khanzode attended her promptly. She was cold, breathing was laborious and there was no BP. The moment she was taken on bed, she had a cardiac arrest. Dr Swapna and her team began chest compressions rigorously and she was promptly put on ventilator. About one hour of CPR and her heart rate was restored. Meanwhile her heart scan showed Massive Pulmonary Embolism ( huge clot in the right side of the heart). Immediately she was given medications for the clot to get dissolved. However her condition remained critical requiring high dose medicines to support her body.

In spite of right kind of treatment, Massive Pulmonary Embolism has a reported death rate of more than 15%, Dr Swapna informed. Timely help from Dr Abhay Thakre, cardiologist was invaluable. However this was just the beginning. The risk of injury to other organs especially brain was still high in spite of timely resuscitation because clot in the right side of the heart stops the forward blood flow from the heart and hence reduced blood flow to organs like brain, kidneys etc is inevitable. After 48hrs, heart started showing recovery, but kidneys got affected, hence dialysis was started. Fortunately MRI brain did not show any major changes due to low oxygen supply and after 72 hours, she started responding.

Dr Swapna said this was the most positive consequence. In medical fraternity, there is a popular saying.."Time Is Brain". But in Shivani’s case, we feared inspite of the fact that CPR was started immediately because of the time taken for her revival. 60 mins is a time enough for brain to get damaged. But fortunately we managed to save it.

One more interesting turn in this case was a rare entity that was diagnosed, called as Paroxysmal Sympathetic Hyperstimulation syndrome. Dr Swapna said, It is an uncommon entity to be seen after cardiac arrest and very few cases of post cardiac arrest autonomic instability are reported. This condition is commonly seen after traumatic brain injury. It's problem is delay in its recognition as signs and symptoms overlap with many other critical conditions seen in ICU. She said, it was a learning curve for us in ICU too. This condition presents with high heart rate, unusually rapid breathing, profuse sweating and pupils become dilated and there is no reaction to light. This can be like a nightmare for the intensivists as it mimics severe brain injury ( eg: bleeding in the brain or raised pressure inside the brain).However these signs are temporary. After thorough evaluation and discussion with Dr Vasant Dangra, Neurologist , diagnosis of paroxysmal sympathetic hyperstimulation was done and accordingly treatment was ensued. She responded to the treatment well and such episodes eventually ceased. This is one important syndrome to be kept in mind by all ICU doctors, especially after reviving a patient from near death situation.

Shivani was recovered completely without any neurological deficit, fully resolved blood clot in lungs and normal heart and totally recovered kidneys. Efforts of doctors from KRIMS Hospital, Nagpur, mainly the ICU team led by Dr Swapna Khanzode along with support of Dr Abhay Thakre and Dr Vasant Dangra paid off well. What's more! Shivani walked back home smiling. Shivani and her family were all praises for the ICU team.

Pulmonary Alveolar Proteinosis (PAP) is a rare lung disease

Pulmonary Alveolar Proteinosis (PAP) is a rare lung disease. Its incidence rate is one case per 5 million people. It is caused due to the accumulation of protein and lipid material, called surfactant, within the lungs leading to reduced oxygen gas exchange.

This condition is diagnosed by doctors through a variety of tests including chest x-rays, breathing tests, and lung biopsy. Medications are not very effective in improving symptoms in patients of PAP. Whole Lung Lavage (WLL) is the current standard treatment for PAP.

A 60-year-old lady had presented to Pulmonology Department, KRIMS Hospitals with symptoms of breathlessness. She was investigated and her CT Scan was suspicious of PAP. On lung biopsy she was proven to be a case of PAP, a disease of very rare occurrence. She was unable to maintain oxygen saturation despite supplemental oxygen. The team of doctors from KRIMS including Pulmonologists, Anesthetists and Physiotherapists planned to perform WLL for this patient. On 30th September, she underwent WLL for left lung while her right lung was mechanically ventilated. A total of 2 liters of the proteinaceous fluid was successfully removed. The whole lung lavage procedure took 3 hours. The patient was discharged with improvement in her health and on chest X-ray. A repeat lavage is planned for her in the next month.

Yet another successful operation at KRIMS Hospital

At KRIMS a 44 years female patient having menorthagia causing Haemoglobin drop to 6 gram was found to have a big Uterine fibroid upto umbilicus Patient weighed more than 85kgs.

She was taken for Abdominal Hysterectomy through Transverse Pflanesteil incision after correction of Anemia.

The chief surgeon was Dr. Meena Patil assisted by Dr. Yamini Alsi and Dr Rita Gupta was the anesthetist. A big fibroid of more than 15cm was removed with conservation of ovaries.

Patient was discharged on 3rd day uneventfully.

60 year old female having 10 years of menopause got pain in abdomen and was found to have a large cystic tumour arising from the pelvis. CT abdomen revealed a more than 15 cm Right Ovarian Tumour which was cystic with projections inside. However the cyst was from surrounding structures. CA 125 value was high i.e. 45.

Patient underwent Laparotomy with Transverse incision. Tumour was removed with Total Hysterectomy by Dr.Meena Patil (Gynaecologist), Dr.Yamini Alsi (Gynaecologist) & Dr.Daksha Mandhaniya (Anaesthetist) & team. Operation was uneventful. Patient was discharged on the third day. Histopathology did not find any malignancy.

The symposium was diligently conducted wherein doctors presented their expert views on various topics. Dr.Ashok Arbat (Vice President - VHA) & his team explained in detail about the Government Schemes (its deficiencies & its solutions) & also about the Social Insurance Schemes.

KRIMS Hospitals conducts National Pulmonology Conference in Nagpur.

KRIMS Hospitals organized its second national conference in Pulmonology, “COPD-National Update 2017” on 26th Feb 2017 at Hotel Radisson Blu. This was the second national conference after the highly successful “ILD-National Update 2016”.

The conference was organized with the aim of bringing stalwarts of pulmonology from all over India to Nagpur so that they share their knowledge with doctors & postgraduate students. KRIMS Hospitals aims to put Nagpur on the national map in the field of research and academics.

The conference started with a Quiz for various Postgraduate students from medical colleges from in and around Nagpur. It was followed by an informative talk by Dr. Sundeep Salvi elaborating on hazards of Smoking. Other speakers included Dr. Raja Dhar from Fortis Hospital Kolkata, Dr. R. Vijaikumar from Apollo, Hyderabad, Dr. Ashok Arbat ,KRIMS Hospitals , Nagpur, Dr. Prashant Chhajed from Fortis Hospitals, Mumbai and Dr. Syed Tousheed from Narayana Hrudalaya, Bengaluru.

COPD is a disease in which patients have breathing difficulty and cough. According to World Health Organisation(WHO) COPD is the third largest cause of death all over the world . In India 30 million people suffer from COPD and 3 million people die due to COPD every year. According to government of Maharashtra it is the number one cause of death in our state. Most of the patients of COPD approach the doctor in the last stage of the disease. Smoking, cooking on chulha and Industrial pollution are the main causes of COPD.

Dr. Sundeep Salvi from CRF, Pune stated in his lecture that, “One session of smoking hukka is equivalent to smoking 100 cigarettes”. Dr. Ashok Arbat, the Organizing Chairman of this conference has been practicing pulmonology for the past 35 years and is referred as a “Pioneer in pulmonary medicine in Central India”. He is deeply dedicated towards creating awareness in the society. Dr. Sameer Arbat, Organizing Secretary of the conference said that, “We have more than 12,000 patients visiting our OPD every year, out of this around 4000 patients suffer from COPD. Every Tuesday we conduct a free of cost patient education clinic for the patients and their relatives. We educate patients about prevention and management of COPD. Only with public awareness, we can control this serious disease”

COPD-NATIONAL UPDATE 2017 was attended by more than 200 delegates including Doctors from Kolkata, Aurangabad, Pune and Nagpur. KRIMS Hospitals plans to conduct a Third National Conference next year.

During the routine interaction of pregnant patient with their Gynecologist, many things are left out in discussion due to lack of time. That is precisely the reason why ‘Gynecology Department’ of KRIMS Hospital arranged a novel programme on “Pregnant Women – Today Something Different” at KRIMS Hospitals on 28th May 2017 (Sunday).

Gynecologists Dr. Meena Patil, Dr. Yamini Alsi, Dr. Anjali Ramakrishnan, Dr. Swati Sarda were prominently present for the program alongwith Dr. Prajakta Borkar (Physiotherapist) & Ms. Shrutika Sarode (Dietician).

Dr. Prajakta Borkar (Physiotherapist) aptly elaborated about the exercises to be done during pregnancy & its benefit to the pregnant women. The importance of nutrition – what to eat, what not to eat & how many times to eat in a day, was discussed at length by Ms. Shrutika Sarode (Dietician).

Dr. Meena Patil (Gynecologist) took time to advocate the pregnant women about how to keep themselves and their baby calm and quiet. "Case of Tracheal Stenosis" treated at KRIMS Hospitals.

Advancements in medical field have paved way for affordable treatment with faster recovery and better outcomes. Recently a similar example was witnessed at a city hospital in Amravati where a 38 year old man was referred with a condition of subglottic stenosis. He had a history of road accident followed by traumatic intubation which led to stenosis of trachea beyond the vocal cords. CT scan revealed that and the stenosis was 5 cm long and was less than 6 mm in diameter. A team of Interventional Pulmonologists at KRIMS Hospitals which included Dr Ashok Arbat, Dr Vinit Niranjane and Dr Sameer Arbat decided to undertake a procedure of Tracheal Recanalisation with stent placement. The Recanalisation was successfully achieved by using the techniques of Interventional Pulmonology. With the Experienced hands and team work of Dr Ashok Arbat, Dr Vinit Niranjane and Dr Sameer Arbat, Dr Swapnil Bakamwar and anesthetist Dr Daksha Mandhaniya, the patient is now able to breathe properly.
